Specialist Consultation Process

Dr. Charl Els, addiction psychiatrist, is available for telephone consultation regarding challenging tobacco addiction cases. TRaC II enrolled physicians may access Dr. Els by the following steps:

  1. Using your standard fax referral form (or by downloading our pre-formatted referral form), please ensure to include the following: 
     
    • Brief outline of the patient’s medical conditions and diagnoses 
    • Current medications 
    • Clinical question(s) 
    • Optimal day, time and phone number for Dr. Els to call (a few options, if possible)
    • Estimated length of consultation appointment 
    • Please mention TRaC II in the referral. 
       
  2. Fax consult to:

    c/o Paige Glennon (780) 425-4274 
     
  3. Dr. Els will contact the physician and complete the consultation.
