About Trac II

Tobacco use remains the leading preventable cause of death, disease and disability. One in five Albertans (20%) smoke but very few receive evidence-based care for their addiction, now recognized as a bona fide chronic relapsing medical condition.

TRaC II provides primary care physicians with a clinical process to integrate tobacco (nicotine) addiction treatment into their practice. It follows the 5A (Ask, Advise, Assess, Assist and Arrange follow-up) Clinical Practice Guideline approach and addresses safety considerations (e.g. mood changes and risk of harm) associated with tobacco cessation.

Through participation in TRaC II, primary care physicians and their teams across Alberta have successfully implemented individually tailored processes for assisting and supporting their patients who wish to stop using tobacco products. These processes focus on the incorporation of clinic team members to share responsibility for such aspects of care as patient education and arranging follow-up. As well, local PCN and community programs are integrated whenever possible.

Who can participate?

Enrollment for TRaC II has closed. However, we are currently seeking sustainable funding, with the goal of offering TRaC II to all interested Alberta primary care physicians and their teams. Check the website regularly for updates.

Privacy and Protection

Toward Optimized Practice (TOP) and TRaC II respect the privacy of participants and are committed to protecting personal physician information. No personal patient information will be collected.
